An excavator operator fell unconscious after leaving his vehicle to clean a pump, landing face-down in contaminated leachate. When his supervisor attempted a rescue, he fell unconscious twice before managing to escape and call for help.
The supervisor developed incapacitating post-traumatic stress disorder, while the operator suffered chemical burns to his eyes, chemical pneumonitis, atrial fibrillation, and seizures. WorkSafe New Zealand’s investigation revealed a familiar pattern: dangerously high levels of toxic gas had been identified weeks earlier, but workers were never informed. No gas monitors were available on site, and the excavation work lacked proper risk assessment or planning.
